ColoWatch® SMS Notification Consent Form

ColoWatch® provides remote radiology reading services to healthcare facilities nationwide. To maintain our high standards of patient care and ensure timely readings, we have implemented an automated notification system to keep our radiologists informed of their case assignments and updates.

Purpose of Communication

We use SMS text messaging as a critical communication tool to notify radiologists of important case-related updates. This system is designed to:

• Ensure timely patient care through prompt notification of new cases
• Optimize workflow efficiency
• Maintain clear communication channels for case management

Types of Notifications

As a radiologist with ColoWatch®, you will receive SMS notifications for the following:

• Assignment of new cases
• Confirmation when cases are ready for reading on your workstation
• Updates regarding case reassignments

Message Frequency and Timing

• Messages will be sent 24/7 in alignment with our continuous operation schedule
• Frequency varies based on case volume
• You can expect between two and four messages per assigned case through its lifecycle

Technical Information

• Messages will be sent from our secure messaging system
• Each message will be clearly identified as coming from ColoWatch®
• Messages will include case identifiers but no protected health information (PHI)
• System status updates will be clearly marked as such

Consent to Receive SMS Messages

By completing and signing this form, I consent to receive SMS text messages from ColoWatch® at the mobile number provided below. I understand that:

• These messages are an essential part of my work with ColoWatch®
• Standard message and data rates from my mobile carrier may apply
• ColoWatch® will only use this number for work-related notifications
• This consent remains valid throughout my contract period unless revoked
• Message frequency will vary based on case volume and assignments
• I am responsible for maintaining a functioning mobile device capable of receiving SMS messages
• I will notify ColoWatch® immediately if my mobile number changes
• Technical issues with message delivery should be reported to ColoWatch® Support

Acknowledgment

I acknowledge that:

• I am the owner or primary user of the mobile number(s) provided
• I am authorized to consent to receive messages at this number
• I understand these notifications are crucial for patient care
• I will maintain the confidentiality of any case-related information received
• I can opt-out of these messages by submitting a written request to Operations
• Opting out may affect my ability to fulfill my obligations
